MMTA’s MUSIC CONNECT PROGRAM

TEACHER RECOMMENDATION FORM

MMTA’s Music Connect Program (MCP) offers music lessons tuition-free to students who have demonstrated financial need and will study with an MMTA member in good standing.

Applications are accepted for the academic year starting July 15 through September 12, and will be considered in the order they are received. Teachers may apply for one student only.

This Recommendation Form is to be filled out by MMTA Member Teachers ONLY.
